Registrar Forms & Services

The MCC Registrar's Office serves students, staff, faculty and the community by providing exceptional support and service in the following areas:

  • Processing and maintaining accurate, detailed and updated student records pertaining to credit courses
  • Processing of transcript requests and transcript evaluations
  • Providing academic enrollment verification information
  • Processing and confirming graduation applications

GED transcripts and diplomas must be sent to the appropriate department requesting them. The Registrar's Office does not retain GED credentials to forward to our Financial Aid Office if there is no indication of where they need to be sent to.

Eligible educational institutions file Form 1098-T for each student they enroll and for whom a reportable transaction is made. Educational institutions file this form for each individual that has made payments for qualified tuition related expenses.

Section 6109 of the Internal Revenue Code requires you to give your correct SSN to MCC since we must file an information return (1098-T) with the IRS. The IRS uses the social security number for identification purposes and to help verify the accuracy of your tax return. If you fail to furnish your correct SSN to MCC, you are subject to a penalty of $50.00 from the IRS.

Instructions

Call the MCC Contact Center at 531-622-5231 to verbally give your SSN.

More Information about the form:

If a 1098-T form is NOT available on Self Service, there may be several reasons that a 1098-T was not generated for a student:

  1. The student did not have tuition charges in the calendar year.
  2. The student did not have an address in Colleague at the time the 1098-Ts were generated. The student would need to update the address, and then contact Student Accounts to request a 1098-T be generated.
  3. The student's information in Box 1 (QTRE - qualified tuition payments received) and Box 5 (financial aid, agency billing, tuition remission, etc.) were equal and netted to a zero amount.
  4. The student is an international student and did not provide MCC with a social security number. If you are an international student and do have a social security number, contact Student Accounts to request a 1098-T be generated.

General Information about the 1098-T form

The Form 1098-T may be used to verify that you are eligible to claim one of the tax credits described below. MCC is required to send a copy of this form to the Internal Revenue Service in order to notify them of your potential eligibility to claim the credit.

Box 1 has been filled for Qualified Tuition Payments Received. Prior to the 2018 calendar year, the MCC 1098-T included a figure in Box 2 that represented the qualified tuition and related expenses (QRTE) MCC billed to your student account for the calendar (tax) year. Due to a change to institutional reporting requirements under federal law, beginning with tax year 2018, MCC will report in Box 1 the amount of QTRE you paid during the year.

The American Opportunity Credit is a refundable tax credit for undergraduate college expenses. It is a tax credit of up to $2,500 of the cost of tuition, fees, and course materials paid during the taxable year as long as the student attended college at least half-time (Box 8). Taxpayers will receive a tax credit based on 100 percent of the first $2,000, plus 25 percent of the next $2,000, paid during the taxable year for tuition, fees, and course materials. Required course materials, books and school supplies are an eligible expense for this credit only. See IRS Pub 970 and/or your tax advisor for other criteria.

The Lifetime Learning Credit applies to tuition and fees for all postsecondary education and for courses to acquire or improve job skills. A family can claim on its tax return a credit equal to 20% of the first $10,000 of adjusted qualified education expenses. The maximum Lifetime Learning Credit you can claim on your return for the year is $2,000. There is no limit on the number of years the Lifetime Learning Credit can be claimed for each student.

It is the student's responsibility, with the help of their tax advisor, to determine the amount that should be reported on Form 8863. Metropolitan Community College does not provide tax advice to any taxpayer or student. Additional information can be obtained by calling the IRS at 1-800-829-1040, or by accessing their web site at www.irs.gov. Publication 970, "Tax Benefits for Higher Education," explains the provisions of each credit in detail.

Students who need to present MCC academic credentials (diploma or transcripts) for use abroad must follow the steps below. All fees involved with authenticating academic records are paid by the student.

Step 1: Order a copy of a diploma and/or transcripts

Step 2: MCC Notarization

The diploma or official transcript is notarized by the MCC Registrar’s Office and mailed to you.

Step 3: State Authentication

You may request a Transcript Re-Evaluation (student login required) if you changed to another program of study or wish specific courses to be reviewed again.

International transcripts sent to MCC must first be evaluated and interpreted by any one of the current members of the National Association of Credential Evaluation Services. A course-by-course evaluation must be requested from these organizations, and the official report must be sent directly to the MCC Registrar's Office. International transcripts will also be reviewed under MCC transcript evaluation guidelines and policies.

MCC credit is awarded for some general education courses to students who have an official transcript on file in the Registrar's Office, noting conferred bachelor’s, master’s, juris doctor, or doctoral degrees from a regionally, accredited American institution. Qualifying students will receive full credit for associate of applied science general education requirements.

Important: Some programs have specific requirements for general education courses. In such cases a transcript will be evaluated on a course-by-course basis. Consult your advisor to see which general education credits you may be eligible to receive.

  • Statement must contain the resolution the student is requesting.
  • Complete documentation to support the appeal request. Proof of documentation may include:

    • Obituary or funeral program.
    • Copy of airline ticket, if travel was required due to emergencies.
    • Official letters or other supporting documents on office letterhead, including the title and phone number of the professional writer of the letter. The letter needs to clearly state the reason why you were not able to successfully complete your courses.

Please include all information with this appeal. If supporting documents are not submitted, a decision will be rendered with the existing information provided.

The Appeal may be submitted via email to: studentrecordappeal@mccneb.edu or via mail to:

Military-Connected students must submit an appeal to the MCC Military and Veterans Services office or by calling 531-MCC-2330.

Appeal Guidelines

  • Personal hardships or failure to read College published documents generally do not present justifiable reasons to support an appeal.
  • Appeals are approved on a case-by-case basis.
  • An appeal for an issue that is more than a year old cannot be made.
  • It is the student's responsibility to demonstrate an unanticipated or unavoidable event, which was beyond the student's control, resulting in the student's inability to complete the quarter or withdraw from courses.
  • Appeals for late withdrawal or a "W" for a course, will ONLY be considered when the course withdrawal period has passed for the appealed course(s). Courses cannot be deleted from transcripts.
  • Appeal decisions will be made solely on supporting documentation provided.
  • Final decision of the appeal will be sent to the student via mail. It is the student's responsibility to keep all contact information current in the College system.
  • Appeals are processed in the order they are received and decisions are made within 4-6 weeks from the time of receipt.

If you have any questions concerning your Student Record Appeal, email:
studentrecordappeal@mccneb.edu. Please be aware that the decision of the Student Record Appeal Committee is final and submissions for a re-appeal will not be considered.

Some records housed in the College Archives, such as timecards, payroll records, purchase orders and student records, are considered confidential and will only be made available to College departments with the originating department's consent.
